Before the mobility 2019/20

Below, find information for the UL students/recent graduates undertaking the traineeship within the Erasmus+ programme in the academic year 2019/20.

  1. Please fill in and print out zaświadczenie-EHIC and submit this document to the IRO (Room No. 6) in order to get the signature. Then you are able to apply for the European Health Insurance Card. It is not obligatory to have the EHIC but otherwise you are obliged to have a private health insurance valid in the host country for the traineeship period and the travel days from/to the host country.
  2. It is mandatory to have the liability insurance coverage (covering damages caused by the student at the workplace), the accident insurance coverage related to the student’s tasks (covering at least damages caused to the student at the workplace), additional medical expense insurance. If the receiving institution does not provide the trainee with abovementioned insurances, the outgoing student is obliged to purchase them on his/her own.
  3. Please fill in the Bank-Account-Details and submit it to the IRO (Room No. 6) not later than on the day when you are signing the grant agreement before your departure.
  4. Please ask for the student/recent graduate status from the Dean’s Office confirming your status during your traineeship period.
  5. If applicable, please ask your Dean to get the written consent to undertake the traineeship during the study period.
  6. Please purchase your ticket to the country of your traineeship. After the mobility you will be able to apply for the partial refund of travel costs by filling in the following document: Travel cost declaration.
  7. Please fill in the Online Linguistic Support test before the mobility. The access to the OLS will be given to the outgoing student before signing the Erasmus+ financial agreement once the student confirms the dates of his/her traineeship period. Firstly, you are asked to send the completed Template of email: OLS-STUDIA-English to the following address: gabriela.szkup@uni.lodz.pl. 
  8. If you are not a citizen of the EU and your traineeship period is longer than 90 days or the traineeship is realized in countries outside the Schengen zone, please remember to apply for the national visa of the host country. Note on visa for participants in Erasmus+ and European Solidarity Corps (002)

You are obliged to sign the Erasmus+ financial agreement in the IRO (Room No. 6) approximately 1-2 weeks before the departure. In order to do this, you must submit all required documents listed below:
– Confirmation of student/recent graduate status at the University of Lodz for the traineeship period (issued by the Dean’s office)
– the written consent of the Dean to undertake the traineeship within the Erasmus+ (if the traineeship period is during classes at the UL)
– Letter of Acceptance from the receiving Institution (it can be a printout of the invitation letter/email sent to the student by the receiving institution) with precise dates  (dd/mm/yy) of traineeship mobility abroad
– a copy of the European Health Insurance Card or other private health insurance valid in the host country for the traineeship period
– a copy of liability insurance coverage (covering damages caused by the student at the workplace), the accident insurance coverage related to the student’s tasks (covering at least damages caused to the student at the workplace) and additional medical expense insurance.
– a copy of the national visa of the traineeship country (if the trainee is not a citizen of the EU and will do the traineeship longer than 90 days or not in the Schengen country).

Without any of the documents listed above, you will not be able to sign the Erasmus+ financial agreement.

The signing Erasmus+ financial agreement is a prerequisite for undertaking the Erasmus+ traineeship and receiving the Erasmus+ grant.
